Home and Community Care Support Services Champlain is one of 14 Home and Community Care Support Services organizations in Ontario. We are dedicated to ensuring our more than 64,000 patients annually receive the highest-quality health care and services at home. Our patient-care coordination teams help develop care plans for people of most ages, focused on maintaining each patient's independence and dignity. We also provide long-term care home placement services, and facilitate our patients’ access to the community-based services they need.

Position Summary:

The Social Worker identifies the need for the delivery of Social Work services, recommends and participates in the development of programs to meet these needs and provides consultation and social work services to selected patients.

Primary Responsibilities:

•Provides social work intervention to referred patients and families, including assessment of psycho-social needs. Refers patients and families to community agencies as appropriate.
•Establishes treatment goals with respect to identified problems and develops methods of achieving these goals in consultation with Care Coordinators.
•Ensures that patient files are initiated, maintained and updated and maintains treatment records in accordance with established guidelines and best practices.
•Ensures that recognition and response to identified social needs by recommending and participating in the development of guidelines and developing resource material.
•Establishes and maintains liaison with other health and social service providers.
•Participates in committees as approved.
•Interprets the role and objectives of the LHIN Social Work Program to the community and interested agencies.
•Partakes in professional development activities and maintains knowledge and ability in the field of social work.
•Identifies gaps in community social services that affects the psycho-social functioning of the patient and family and participates in improving the services offered

Education and Experience:

•Master’s degree in Social Work and current certificate of registration with the Ontario College of Social Workers and Social Service Workers
•Related professional experience which would normally be attained over a period of two years in a similar (Social Worker) role
•Related experience in a similar community-based setting is an asset
•Experience providing service to patients in the area of stroke rehabilitation is an asset

Knowledge:

•Knowledge of medical conditions and cognitive/psychiatric disorders
•Knowledge of health care system and the role of the HCCSS within the system
•Knowledge of stroke rehabilitation evidence-based practice
•Knowledge of relevant health care legislation
•Knowledge of HCCSS policies, procedures, strategic direction and goals
•Understanding of HCCSS partners and contracted service providers in the community

Skills and Abilities:

•Strong interpersonal skills to work with diverse patient groups with varying levels of comprehension and language capability
•Strong ability to manage difficult conversations/situations in a healthy and effective manner
•Demonstrated ability to work effectively in both a team environment and independently
•Effective communication skills to establish and maintain a wide range of contacts with professionals and organizations within the community
•Strong customer service skills
•Effective listening and strong facilitation skills are necessary to work with patients and their families/caregivers/SDMs in order to ensure best possible service is provided to the patient
•Ability to deliver information effectively in a variety of settings including one-on-one, team meetings, and to assist in delivering presentations/information regarding the LHIN Social Work program
•Effective planning and organizing skills to manage multiple patients, provide information reports, and take corrective action as required
•Ability to accurately complete required documentation, reports, forms, and recommendations
•Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel, Power Point, etc.)
•A valid driver’s license and access to a vehicle is required
•Vulnerable Sector Check completed within the last six months

Pay rate: Wages are in accordance with the collective agreement between Home and Community Care Support Services Champlain and OPSEU and starting salaries range from $42.934/hr. - $49.857/hr
Shift: Monday to Friday; 8:30am - 4:30pm
